Participant Waiver + Liability Release

I, the undersigned parent or legal guardian, acknowledge that participation in the PrimeTime × Troy Henderson Basketball Camp involves physical activity and inherent risks including:

• Physical injury from athletic activity including sprains, strains, and fractures
• Slips, falls, or collisions with other participants or equipment
• Heat-related illness during athletic activity
• Pre-existing medical conditions that may be aggravated by physical activity

I understand these risks cannot be entirely eliminated and voluntarily choose to have the athlete participate with full knowledge of these risks.

In consideration of the athlete's participation, I hereby release, waive, and discharge True Hero Sports Group, LLC, PrimeTime Developmental Camp, Troy Henderson, Donte Henderson, Northumberland High School, and their respective officers, employees, volunteers, and agents from any and all claims arising out of the athlete's participation, including claims arising from negligence, to the fullest extent permitted by law.

I authorize camp staff to obtain emergency medical treatment for the athlete if I cannot be reached. I am responsible for all medical costs incurred. I certify the athlete is physically fit to participate and have accurately disclosed all relevant medical information during registration.

I agree the athlete will treat all participants and staff with respect, follow all staff instructions, refrain from unsafe or disruptive behavior, and adhere to all facility rules. Failure to comply may result in dismissal without refund.

I assume all risks associated with participation and agree to indemnify and hold harmless the Released Parties from any claims, liabilities, or costs arising from the athlete's participation.

I grant True Hero Sports Group, LLC the right to photograph and record video of the athlete during camp for use in social media, marketing materials, and future camp promotion. If you do not wish to grant this permission, notify us at trueherosports@gmail.com before camp.

By completing online registration and checking the acknowledgment box, I confirm my electronic signature is legally binding and has the same force and effect as a handwritten signature.

This agreement is governed by the laws of the Commonwealth of Virginia.
